本實習課程乃建構於學生,實習機構與學校三方面的合作關係,各方皆有其應盡之責任,義務與功能,方能達成各方預期之目標,於此證明書簽署後,若任何一方未經其他兩方同意,不得擅自更改契約內容,否則本實習課程將視同無效.
