要求

首先，假條的上方還是應該按照半正式的格式寫上如下資訊：

To：假條是遞給誰的

From：請假人

Date：寫假條的日期(注意不是請假的日期)

Subject：寫上請假字樣

其次，在您的請假信第一段，應該開門見山但是有禮貌地提出請假。第一段要中心明確，寫清您要請假的日期。

然後，在假條的第二段，您應該簡單明瞭陳述請假的原因事由。注意敘述清楚明白，並儘可能表示對此帶來工作不便的歉意。(比如原因就寫打球后回家洗澡感冒。)

接著，在假條的最後一段，應寫上您希望獲得准假的句子，或者具體等候答覆的時間。

首先，要在假條的上方寫明假條是遞給誰的

To：假條是遞給誰的

From：請假人

Date：寫假條的日期(注意可不是請假的日期，不要弄混了哦!)

Subject：寫上請假字樣

例如：

To: Peter Stone, Manager

From: Lynn Chen, Financial Department

Date: May 22nd, 2019

Subject: Casual Leave of Absence

其次，在假條的第一段，要開門見山但是有禮貌地提出請假，要寫清你要請假的日期。

例如：

Peter, I would like to know if I could ask for a casual leave of absence for one day on May 24th, this Wednesday.

然後，在假條的第二段，你要簡單明瞭地陳述請假的原因事由，並且要表達對此給工作帶來不便的歉意。

例如：

This morning I received a telephone call from my dentist, urging me to come to his practice for immediate treatment of my teeth. I have been experiencing a stinging pain, depriving me of my sleep during the past fortnight. The situation could worsen, should infection occur.

Concerning my workload: As Wednesday is not as busy as the other weekdays, I think a one-day leave this Wednesday may be the best solution. I apologize for the inconvenience my absence from work may cause.

接著，在假條的最後一段，要寫上你希望獲得准假的請求，或者具體等候答覆的時間。

例如：

Thanks. I will call you at 1:30p.m. Or you can call me at any time.
